Hello, my name is _____. I came here because I/a friend was banned. I would like to know which rules I/he/she broke, and if it is possible for me/him/her to be un-banned. My/his/her in-game name is _____ and was banned on/around __/__/__.

I apologize for the rules I/he broke. And if it is possible to be un-banned, would you please do so?
